Program Completion Requirements

Interns must complete all required rotation experiences and assignments at a level of 4 or 5 and they must all be verified with the appropriate preceptor signatures and a minimum of 1200 supervised practice hours within 31 weeks.

All evaluation forms must be complete and signed by preceptors and on file in the Program office. All other documents required by the Program must be complete and on file in the Program office, including an exit survey. All written work and reports to the Program office must be submitted to the Program office. In special circumstances (medically related or major life event related absence only), as defined in the Intern Handbook and Policies and Procedures, an Intern will be allowed up to 36 weeks from the scheduled start time in place at the time the student enrolls to complete the program.
